Clinical Study of the Correlation between Histological Types and the Efficacy of mFOLFOX6 ± Bevacizumab in Patients with Metastatic Colorectal Cancer

Oxaliplatin based chemotherapy has been commonly administered for metastatic colorectal cancer (mCRC), but there is no important predicting marker except for K-ras mutation. We studied the correlation between some prognostic factors (CEA, number of metastases, and histological types) and the efficacy of oxaliplatin based chemotherapy in patients with mCRC. We analyzed 69 consecutive patients, involving 31 patients treated with mFOLFOX6 and 38 patients treated with mFOLFOX6 + bevacizumab. Histological types of primary tumor were divided into three groups: TUB1, TUB2, and poorly + mucinous (MUC + POR). Comparisons were made with regard to response rate (RR), progression free survival (PFS) and overall survival (OS). In the 69 patients, TTF was 5.5 months, PFS was 9.1 months, and MST was 27.9 months. RR of histological types was as follows: TUB1 group (22 cases): 50.0%, TUB2 group (34 cases): 52.9%, MUC + POR group (13 cases): 7.7% (vs. TUB1: p=0.013, vs. TUB2: p=0.007). OS showed significantly poor prognosis in the MUC + POR group (vs. TUB1 + TUB2: p=0.0054). RR and OS of the MUC + POR group showed significantly poor prognosis compared with other histological types. Although further analysis is needed, we should consider histological type in the therapeutic strategy for mCRC.
