A Case of Incessant Ventricular Tachycardia in a Patient with Cardiac Sarcoidosis and Paroxysmal Atrial Fibrillation That Was Successfully Terminated by Pulmonary Vein Isolation

A 64-years-old man who received an ICD implantation for ventricular tachycardia (VT) due to cardiac sarcoidosis was referred to our hospital because of electrical storm. He had been treated for paroxysmal atrial fibrillation (PAF) since 2004. Furthermore, she suffered with palpitation and near syncope due to non sustained VT. He was diagnosed with cardiac diagnosis accompanied by PAF and VT and received an ICD in 2005. PAF and VT were eliminated by receiving oral amiodarone. However, PAF episode worsened since 2011. PAF was associated with frequent delivery of ICD therapies. Electrogram stored in ICD showed that VT episode was always preceded by PAF. We planned catheter ablation to VT. However, we could not obtain a delayed potential or other appropriate electrogram for ablation in ventricle. Therefore, we performed pulmonary vein isolation (PVI) to eliminate PAF that was an important role of VT trigger. No etectrical storm and VT was recognized during 12 months after an achievement of PVI.
