Improvement of TWA after Comprehensive Cardiac Rehabilitation in Patients with Acute Coronary Syndrome or Low EF

<B>Background:</B> T-Wave Alternans (TWA) is used to evaluate the risk of arrhythmia events in patients with structural heart disease (SHD). However, whether TWA could be improved by the comprehensive cardiac rehabilitation (CCR) in patients with SHD, still remains unknown. <B>Method:</B> This study comprised 232 pts. They included ACS (n=124), stable angina pectoris (n=91), or chronic heart failure (n=17). We investigated the max score of TWA (time domain method) during CPX exercise testing before and 4 months after the CCR respectively. <B>Results:</B> The max score of TWA significantly decreased in the whole patients (24.15±12.96 vs. 21.52±11.36, P<0.05), and the patients with ACS (26.12±14.69 vs. 21.65±12.17, P<0.05), the patients with low EF (EF<50%) (26.95±13.75 vs. 21.80±10.98, P<0.05, n=66). But it did not differ in patients without ACS (21.89±10.25 vs. 21.37±10.40, P=NS). In particular, there were a greater degree of the decrease in patients with both low EF (EF<50%) and ACS (28.20±15.44 vs. 22.04±12.11, P<0.05, n=46). No significant difference was shown in EF between the patients with and without ACS (58.3±11.9% vs. 58.9±12.4%, P=NS). <B>Conclusion:</B> TWA could be improved by CCR especially in patients with ACS or low EF.
