Purpose: The aim of this study was to investigate the target local electrogram to create the conduction block of the cavotricuspid isthmus (CTI). Methods: The study population was 334 consecutive patients (268 male, 60±11 years). We analyzed the local electrogram just before completing CTI-block as well as the changes in the stimulus-distal TA time (S-dTA). Result: Significant relationship between S-dTA prolongation values and the local electrogram amplitude just before successful ablation was observed. In 126 patients (38%), S-dTA was not prolonged until the successful RF application (<10 msec, Group-A). In remaining 208 (62%), S-dTA was gradually prolonged (≥10 msec, Group-B). The local electrogram amplitude in Group-A was significantly higher than that in Group-B (0.63±0.41 mV vs. 0.33±0.31 mV, p<0.0001). Conclusions: The targeted electrogram amplitude just before successful CTI ablation was significantly correlated with the S-dTA time prolongation.
