Background: The purpose of this study was to compare the postoperative results of the rotator cuff repair using the metallic screw type suture anchor (Twinfix anchor) with the results using bioabsorbable hook type suture anchor (Panalok RCDS anchor). Methods: Ninety two rotator cuff tears were repaired arthroscopically with either the metallic screw type suture anchor, or the bioabsorbable hook type suture anchor. The postoperative results were evaluated by the JOA scores and MRI images with the Sugaya's classification at six months after the operation. Results: In the 68 cases using bioabsorbable anchor, the JOA score was improved significantly, and the re-tear rate (type IV and V of Sugaya's classification) was 17.6%. In the 24 cases using metallic anchor, the JOA score was also improved significantly, and the re-tear rate (type IV and V of Sugaya's classification) was 25.0%. There was no significant difference between two groups in the postoperative results. Discussion: The biomechanical cadaveric studies have reported the advantages of the screw type anchors in comparison with the hook type anchors. However, this study showed that in the clinical results, the postoperative results of the hook type anchor had no significant difference from the results of the screw type anchors. Rotator cuff repair using the hook type bioabsorbable suture anchor is a useful operative procedure.
