<B>Background:</B> In 2007, we investigated 56 shoulders and reported that we classified the tear site of the rotator cuff and conducted an investigation into the influence of each rotator muscle strength. In this study, we investigated 80 shoulders, and report the relationship between the muscle strength of abduction, external rotation, internal rotation.<BR><B>Methods:</B> From October, 2004 to December, 2010, we evaluated 80 patients with rotator cuff tear. We defined the location of cuff tear in the facet as zone1;superior facet(zone1a; anterior part of attachment of SSP tendon, zone1b; posterior part of zone 1a), zone2; anterior part of middle facet, zone3; posterior part of the middle facet, and zone4; inferior facet. We measured both side of the muscle strength of abduction, external rotation, internal rotation, on both sides using Microfet.<BR><B>Results:</B> The muscle strength of the unaffected side became significantly greater in the order of abductor <ER <IR strength. There was no tear at only zone1a, and with extension of the tear area, the ratio of tear extension to zone1a increased. When tears were included in zone 1a, there was a tendency to decrease the rate of muscle strength of IR on zone1 and zone12. When tears extended to zone 4, decreasing rate of muscle strength on ER2 test was more than those on ISP tests. With extension of the tear area, muscle strengths of both abduction and ER fell, but relatively that of IR was preserved.<BR><B>Conclusion:</B> 1.There was no tear at only zone1a, and with extension of tear area, the ratio of tear extension to zone1a increased. 2.When tears were included to zone 1a (anterior part of attachment of SSP tendon), there was tendecy to decrease the rate of muscle strength of IR on zone1 and zoen12. 3. When tears extended to zone 4 (attachment of teres minor muscle), the decreasing rate of muscle strength on ER2 test was more than those on ISP tests.
