Six cases of acute stroke which all showed severe neurological dificits were treated by emergency reconstructive surgery.<BR>Embolectomy was performed on three cases within 6, 9. 5 and 18 hours respectively from the onset of stroke, and another three cases underwent STA-MCA anastomosis within 5.5, 9 and 21 hours respectively from the onset.<BR>Hemorrhagic infarction followed all three embolectomy cases and two of them resulted in death. The rest of the cases showed transient exacerbation. Three STA-MCA anastomosis cases enjoyed complete remission and the rest of them resulted in favorable outcome. No hemorrhagic infarction was revealed in all three on CT examination.<BR>The authors conclude that, as far reconstructive surgery for acute stroke, bypass susgery is not only safer but also more fruitful than embolectomy.
