• Neuro-otological examinations were done on six patients with Rebound Positional Nystagmus (RPN) in an attempt to clarify the site of lesion associated with RPN. These six cases included two of olivo-ponto-cerebellar atrophy, two of late cortical cerebellar atrophy, one of Menzel type of hereditary ataxia and one of with a tumor in the fourth ventricle.RPN is rarely seen in direction-changing positional nystagmus. The characteristics of the RPN are as follows; when the patient is placed in a supine position, no spontaneous nystagmus is observed. Then, the patient is placed in either right or left lateral position, i. e., the head is rotated to either right or left side while the patient is supine, the nystagmus is first observed. The direction of the fast component of the nystagmus will change to the opposite direction when the lateral position is slowly returned to the supine position.We found two types of the RPN; type one, RPN found in upward-beating direction-changing positional nystagmus, and type two, RPN found in downward-beating direction-changing positional nystagmus. Our neuro-otological examinations suggest that type two is distinct from type one, although the significance of the difference is not clear at present.Common to all the six cases reported, the lesions centered around the midline structure of the cerebellum, and such was also confirmed by neuro-otological examinations.We therefore suggest that the RPN is closely related to lesions in the cerebellum, especially of the vermis.
