The present study was conducted to determine whether Idebenone reduces gentamicin-induced ototoxicity.Forty guinea pigs were divided into two groups; the treatment group was treated with idebenone and the control group was not. In all subjects 0.2 ml (8 mg) per day of gentamicin solution was instilled into the tympanic cavity for four days. In the Idebenone-treated group idebenone (50 ml/kg/day) was intraperitoneally administered for three days prior to gentamicin administration. ABR examination was performed before and after drug administration. On the day following the last administration of gentamicin, all animals were sacrificed for SEM study.The increase in hearing threshold after drug administration was significantly smaller in the Idebenone-treated group (16.25±20.57 dB) than in the control group (29.75±17.73 dB). A variety of stereociliary pathologies could be observed in both groups, including floppy, fanned-out, fractured, fused, giant, and dissolved cilia. Moreover, in the idebenone-treated group a unique stereociliary pathology which has never been reported before, elongated fungus-shaped cilia covered with a balloon-like bleb, was observed. The development of such fungus-shaped cilia seemed to be the result of the inhibitory effects of idebenone on gentamicin-induced ototoxicity.
