A case of the spontaneous rupture of the brachial triceps tendon in a patient with secondary hyperparathyroidism was reported. The patient was a 33-year-old male who had undergone a hemodialysis for these ten years for a chronic renal failure. Chemical examination revealed elevation of BUN, Cr, AlP and parathyroid hormone, and serial roentgenograms showed renal osteodystrophy. The triceps tendon was ruptured in the vicinity of its insertion to the olecranon. The ruptured tendon was repaired by a Scuderi's technique and it gave a satisfactory results.
