Closed Osteotomy for Tibia Vara

This is a report of cases of tibia vara in infants whom we have treated with an original method of closed osteotomy.Subjects: Infants with tibia vara exceeding 20° whose parents were genetically predisposed to tibia vara. Treatment was limited to children whose parents requested the operation. Method: The area of the medial side of the tibia with the most extreme varus is chosen for osteotomy. After drilling the cortex with a K-wire, valgus osteotomy is conducted manually to induce a form of greenstick injury. A slight external rotation is added. The leg is fixed with a long-leg plaster cast which is removed after 4 to 6 weeks, about the time when callus formation has occurred.Results: Tibia vara that had an average pre-operative angle of 19.5° were reduced to -1.6° immediately after the operation and are presently an average of 0.7°, excluding those who were operated on less than one year ago. The femurotibial angle, which averaged 192.4° preoperatively, was corrected to 169.9° immediately after the operation and is presently 170.7°This method does not leave scars on skin, facilitates rapid bone union, presents minimal side-effects and can be conducted easily at any time and place. This is a safe and sure method of treatment that we would recommend all orthopedic surgeons to attempt as soon as possible.
