Since 1987, we applied Cotrel-Dubousset transpedicular fixation system to lumbar spinal disorders. In 5 cases, it was used for segmental instability associated with lumbar spinal canal stenosis or lumbar disc lesion. It was also applied for 6 cases of spinal malalignment combined with lumbar spondylolisthesis. In a case of L-4 degenerative spondylolisthesis, break of pedicular screw was observed. For segmental instability, best stabilization was obtained by using internal fixation system and postelolateral fusion. Best correction and stabilization of spinal malalignment was achieved by combined procedures of internal fixation and posterior lumbar interbody fusion.We recommend this method as instrumentation of the lumbar spinal disorder accompanied with segmental instability and/or malalignment.
