Transpedicular fixation for non-union or delayed union following anterior spinal fusion of the thoracolumbar and lumbar spine.

Six patients who had unsuccessful anterior spinal fusion for thoracolumbar fractures and degenerative lumbar diseases were treated by transpedicular fixation with posterior and posterolateral bone grafting. In four patients anterior bone fusion was confirmed by disappearance of clear zones observed preoperatively, and in two patients, though clear zones were observed, posterior and posterolateral bone fusion was confirmed by dynamic radiograms during a mean postoperative period of 9.7 months. Instrumentation failure occurred in one patient. We found that satisfactory bone union can be obtained for non-union or delayed union following anterior thoracic and lumbar spinal fusion by transpedicular fixation with posterior and posterolateral bone grafting.
