Posterior spinal instrumentation and spine fusion for unstable fractures and fracture-dislocatins of the thoracolumbar spine.

Ten patients with acute, unstable fracture-dislocations of the thoracolumbar spine were treated with posterior spinal instrumentation and fusion. Nine of the ten patients sustained a neural injury. Seven sutained complete spinal cord injury and two had imcomplete neurological deficits. There were five rotatory fracture-dislocations and three fracture-dislocations and two burst fractures. The Harrington instrumentation was used three times and Harrington-Luque instrumentation was used seven times. Two patients had improved from Frankel Grade C to E post-operatively. The use of Harrington instrumentation is an effective means to obtain the reduction of fracture-dislocation and maintenance of alignment. But a patient with burst fracture had significant loss of correction. Anterior spinal fusion should be recommended to this case.
