Spinal subdural abscess is rare. This time, We report a case of subdural abscess of lumbar cord following infection of upper air way. This patient is a 42 years old man. He caught a cold two weeks before admission. Gradually, he complained of lumbar pain. When he was admitted to our hospital, he could not sleep at night because of severe back pain. Neurological examination revealed mild dysesthesia of both legs and incontinence of urination. Laboratory examination did not show the elevated leukocyte count and positive C-reactive protein. Myelography showed irregular filling deffect between L5 and S1. MRI showed isointensity on the T1 weighted image and high intensity on the T2 weighted image. We could not diagnose subdural abscess from these examinations. On June 8, 1990, laminectomy at the level of L4-S1 vertebrae was performed. Subdural space was filled with pus and scar-like tissue. Culture of this pus yielded a growth of streptococcus milleri. After operation, antibiotics was infused intravenously for 4 weeks. This patient can walk without any crutch. Till now, 44 cases were reported of this disease by our inspection.
