We experienced four cases of extensor tendon rupture following primary osteoarthritis of the distal radio-ulnar joint (DRUJ). They included three males and a female, whose ages ranged from 56 to 80 years (average 69 years).In all the cases, roentogenograms showed osteoarthrotic changes of the DRUJ and dorsal subluxation or dislocation of the ulnar heads.Tendon transfers were done in three cases, in addition to Darrach's operation in one case and a Sauve-Kapandji operation in another case. Friction at the ulnar head and its osteophytes caused the extensor tendons to rupture.
