Nine cases with the spontaneous osteonecrosis of the knee were treated by High Tibial Osteotomy (HTO) between 1983-1991.There were 8 female patients and 1 male patient. The age of these patients ranged from 64 to 79 years old (average age, 70.3 years). The period of follow-up study ranged from 7 months to 7 years (average time, 4.2 years).These cases were classified into stage II (3 cases), stage III (4 cases) and stage IV (2 cases) as defined by Koshino.The radiographical improvement of the osteonecrosis was obtained in 78%, especially in stage II of Koshino classification. The clinical results were satisfactory in the group with the Femoro-tibial angle (FTA) of less than 170°.
