Long-term hemodialysis related arthropathies in 10 shoulders of 8 patients were treated surgically. The pathological foci were almost all in the intertubercular sulcus where the long tendon brachii inserts and the rotator cuff attaches to the greater tuberosity of the humerus. Marked proliferation of bursae and multiple cystic bone lesions were found with amyloid deposition observed histologically in all cases.Surgical treatment such as curettage and bone grafting were quite effective and the clinical symptoms decreased dramatically after surgery. The enthesopathy induced by amyloid deposition is considered to be the main etiological pathology of the hemodialysis related arthropathy in the shoulder.
