Measurement of bone mineral density in defined areas around metal implants has improved with the development of dual energy X-ray absorptiometry. We used this technique to difine the natural history of bone loss around a femoral prosthesis and to determine the various factors which affect the periprosthetic bone loss. The bone mineral density (BMD) around the prosthesis was measured in 15 femurs with total hip arthroplasty. The BMD decreased gradually in all periprosthetic areas of the femur within a few years after surgery. The greatest mean decrease in bone mineral density occurred adjacent to the proximal one-third of the femur. The BMD of lumbar vertebrae in anterior-posterior direction six months post-operatively and the ratio of implant occupation in the femur were also measured to analyze the relationship with the change of the periprosthetic BMD. However, they had no relationship with the change of BMD of the femur during the two years postoperatively.
