This study was made to examine the effect of A-bomb exposure on incidence of pulmonary tuberculosis in the population of the Adult Health Study (AHS) which has been conducted by ABCC-JNIH in Hiroshima and Nagasaki since 1958 and to evaluate the role of AHS in t he management of pulmonary tuberculosis in the examined members.<BR>By the end of the 7th cycle of examinations (30 June 1972), 147 active pulmonary tuberculosis cases were newly detected out of 15, 460 persons who came for examination two or more times (Pleurisy cases were excluded). The annual average incidence rate per 100, 000 person-years during 1958-72 was 161 for males, 56 for females and 95 for sexes combined.<BR>This rate was approximately one-third of that for all Japa n during the same period.<BR>No correlation was found between radiation dose and incidence of tuberculosis. The incidence did not differ between the two cities.<BR>A declining trend of the inc i d ence with time was noted, but the incidence rate in older males increased in the recent years.<BR>A mail survey was c arried out on these cases in January 1973 to learn the mode of detection of their diseases, physicians instructions at the time of detection, and the history of treatment. Thirty-two deceased cases and 5 cases whose addresses were unknown were excluded from the survey. The number of responses received was 88 (80% of the questionnaires mailed).<BR>Fifty-three percent of the responded cases were detected through AHS, but their treatment rate was slightly lower than that for cases detected outside of ABCC.<BR>These results point out that AHS has been playing a usefu l role in the early detection of pulmonary tuberculosis, however, it is desirable that the patients be motivated to receive complete treatment through appropriate post-examination guidance.
