長野保健所管内の新登録結核患者の実態と発見方法 : 第2報面接調査による発見動機

In the Report I of this study, a survey was made on newly registered tuberculosis patients in Nagano Health Centre area in 1972 concerning the severity of disease, mode of detection and the coverage of previous MMR listed on the registration cards. In the present study, presence or absence of symptoms at the time of detection and the mode of detection were investigated through the interview of public health nurses. Interview was made on 148 newly registered patients and 35 relapsed patients. The results were summarized as follows:<BR>1) Diagnosis of tuberculosis was established at public hospitals in 45% of newly registered patients, at private hospitals in 26% and at private clinics in 25%.<BR>2) Tuberculosis was detected by symptomatic visit to physicians in 57% of newly registered patients and in 63% of relapsed patients. The fact indicates that respiratory symptoms are highly prevalent among newly detected patients, and the importance of visit to physicians must be stressed to those who had continuous respiratory symptoms such as cough and sputum.<BR>3) The proportion of those found by MMR was 41% in newly registered patients by the interview, while it was 26% by a survey of registration cards. This discrepancy could be explained by the following facts;<BR>a) Detailed examinations and reporting of new cases detected by MMR in this area were made by the Nagano Tuberculosis Prevention Centre, and some of newly detected cases by MMR had symptoms and they were falsely listed as detected by symptomatic visit to physicians.<BR>b) some cases who were indicated further detailed examinations by MMR made a visit to other physicians and they were diagnosed as tuberculosis, reported from these physicians and were listed as detected by symptomatic visit to physicians.
