PATHOLOGICAL AND MORPHOLOGICAL STUDIES OF PRIMARY LIVER CANCER:HEPATOCELLULAR CARCINOMA AND PORTAL HYPERTENSION

Patients with hepatocellular carcinoma developing remote extrahepatic metastases and portal hypertension seem to be encountered clinically more often in recent years, presumably due to prolonged survival attributable to early diagnosis and chemotherapy. The cause of portal hypertension was investigated from the pathological point of view in 225 autopsy livers bearing hepatocellular carcinoma. It was found that the contributing factors are : 1) compression of small hepatic vein branches by regenerative nodulues of the accompanying cirrhosis, 2) increased arteries supplying the tumor and marked increase of arterial blood flowing into the liver, 3) abnormal communications between the hepatic artery and portal vein through tumor thrombi, and 4) marked disturbances in venous blood flow in the liver due to tumor thrombi and dilated arterio-portal anastomoses through plexuses of capillaries around the bile ducts.
