Percutaneous transhepatic obliteration of gastroesophageal varices with absolute ethanol.

The optimum volume of absolute ethanol was examined, in 29 patients undergone percutaneous transhepatic obliteration of esophageal varices with absolute ethanol when combined with stainless-steel coil, it was in proportional correlation to the diameter of the obliterated vein above 3mm in the diameter. For veins below 3mm in diameter, it was less than 3 ml without coils.<BR>The prognosis and cumulative rate of re-hematemesis were compared between 2 groups, one 29 patients (ethanol group) and the other 24 (glucose group). The rate of re-hematemesis and mortality in the ethanol group, were 13.8% and 6.9%, while in the glucose group, 36.8% and 15.8%. The 2-year cumulative rate of re-hematemesis, in patients treated with PTO alone, was 20.8% in the ethanol group and 49.5% in the glucose group. These rates were almost the same even in cases combined with surgical procedure or endoscopic sclerosis. Absolute ethanol as a embolic material is prooved to be safe and effective even for emergencies.
