In order to clarify the characteristics of early hepatocellular carcinoma of well differentiated type (eHCC), 19 HCCs smaller than 2cm in diameter obtained by ultrasound-guided fine needle biopsy were applied for immunohistochemistry and electron microscopy. Using the silver staining technique, argyrophilic nucleolar organizer regions (AgNORs) have been studied in paraffin sections of eHCC, advanced HCC, and liver cirrhosis. A significant difference was found between the numbers of AgNORs in the nuclei of eHCC, advanced HCC and liver cirrhosis. In 16 cases (82.4%), the endothelial cells were stained with Ulex europaeus 1 lectin (UEA-1), while 11 (57.9%) stained with Factor VIII related antigen (FVIII). The tumor cells of eHCC revealed the ultrastructural features of nuclear change and various intracytoplasmic changes. In the sinusoidal lesion, basement membrane like structure and multi-layered endothelial cells were observed. These findings suggest that ultrastructural characteristics of eHCC is similar to those of advanced HCC and has biological aggressiveness in respect of cellular proliferative activity.
