道内で発生したQ熱の一症例における疫学調査から得られた知見について

In May 2005, one acute case with Q fever occurred in Hokkaido. To identify the origin and the epidemiology of this occurrence, we carried out a questionnaire survey on the patient, and obtained data about population and breeding status of livestocks, a living status of wild animals, birds and ticks around the place where the patient lived, and also obtained geographical and meteorological data around there. For the period from April to September in 2005, no other case of Q fever was reported there. Additionally, perinatal abnormality among animals, such as abortion suspected of being caused by Coxiella burnetii infection, was not found. Therefore, we determined this case as a sporadic one, because outbreak of Q fever had not occurred. To evaluate the level of contamination of C. burnetii around patient's house, we performed serological and/or genome detection survey using samples from dog kept by the patient, 42 rodents captured around there, 66 ticks attached on these rodents, 27 samples of dust or stacked compost corrected around patient's house or sheds. In serological survey, the IgG antibodies against phase II of C. burnetii in dog and rodents were tested by indirect immunofluorescence assay. In addition to these serum samples, ticks, dusts and compost samples were examined for the com1 gene coding for an outer membrane protein of C. burnetii by nested polymerase chain reaction. One of rodents showing antibody titer of 1:32 was considered to be positive, however, the other were negative with titers less than 1:16. On the other hand, all samples were negative for the gene of C. burnetii. The route or source for this acute Q fever was not clarified in this epidemiological survey. These results suggested that the place where the patient lived was not highly contaminated by C. burnetii.
