Immunoadsorption with Tryptophan or Phenylalanine Columns as an Effective Treatment for Pemphigus Vulgalis(Workshop1-12)

Using plasma exchangetherapy, we have treated numerous patients with autoimmune bullous diseases such as pemphigus vulgaris (PV) and bullous pemphigoid (BP), using either centrifugation, double filtration plasmapheresis or a combination of these methods.To further improve the specific removal of autoantibodies, we utilized, for this study, an immunoadsorption methodusing tryptophan or phenylalanine columns. The clinical features and PV titer were observed before and after each procedure.A decrease in PV titer was observed in all patients administrated Imusorba-tryptophan (ITR), while the clinical findings, such as the neogenesisof blisters, improved in eight cases (80.0%). Using Imusorba-phenylalanine (IPH), four patients showed improvement. We were also able to reduce the doseof steroids i n 12 cases. These datastrongly suggestthat ITR is more effective than IPH for the removal of PV antibodies.
