Average life exprctancy has rapidly improved in Japan after the World War II and now become longest in the world. Development of medical technology and improvement of nutritional and hygienic conditions are the major causes of this situation, as well as comprehensive national health insurance system. On the other side, national health expenditures in Japan has increased rapidly as aging society has advanced, and prevention of life-style related diseases and restraint of health expenditures are the major political issues. Furthermore, shortage or uneven distribution of physicians and experts medical doctors, especially pediatricians, obstetrician made difficulties in provision of medical services at local areas after new clinical training program was induced in 2004. As health care system in Japan faces the turning point now, drastic health care reform and establishment of sustainable medical services are strongly aspired. In this article, present state and perspectives of medical services in Japan are discussed.
