To investigate the relationship between apoptosis and expression of bcl-2 protein in tumor cells in both follicular lymphomas and diffuse lymphomas of the B-cell type, immunohistochemical examination was carried out. Sixteen surgically resected gastric malignant lymphomas were examined. They comprised 10 diffuse lymphomas and 6 follicular lymphomas. Immunohistochemically, seven of the diffuse lymphomas and all of the follicular lymphomas expressed bcl-2. In the diffuse lymphomas, bcl-2 was expressed both in lymphocytes intermingled with the tumor tissue and in the tumor cells themselves. Also, in the follicular lymphomas, small lymphoid cells surrounding tumor nodules expressed bcl-2 protein more predominantly than the tumor cells. Nucleoli in apoptotic cells, detected by TdT-mediated dUTP-biotin nick end-labelling (TUNEL), appeared spherical, irregular or fragmented, and were scattered throughout of the tumor lesions. On the other hand, these was no significant quantitative difference in the distribution of apoptotic cells between diffuse and follicular lymphomas. Moreover, apoptotic cells did not express bcl-2. The results of double staining indicated that bcl-2 positive cells did not undergo apoptosis. The present findings suggest that bcl-2 in lymphocytes located in diffuse lymphoma and follicular lymphoma rescues the cells from apoptosis and promotes tumor growth.
