Insulin-like growth factor-binding proteins (IGFBPs) were originally discovered as specific carrier proteins for IGF-I and IGF-II. In addition to the role of IGF-binding proteins, which prolong the plasma half-life of IGFs, recent studies have suggested that IGFBPs are important modulators of IGF action in many aspects. Depending on the experimental conditions, IGFBPs act as not only inhibitors but also enhancers of IGF actions. Regarding this issue, the physiological significance of IGFBPs is discussed. Possible mechanisms for modifying IGF action on the cell surface are as follows: 1) switching the affinity of IGFBP to IGF so that IGF can readily reach its specific IGF receptors, in some cases by extracellular matrix proteoglycans; 2) degrading the intact form of IGFBP by BP proteases, resulting in reduced affinity for IGF; and 3) IGFindependent IGFBP actions on the cell surface. In this paper, some of the currently available reports describing these mechanisms are reviewed. Furthermore, the most updated clinical reports regarding IGFBP expression in the fields of endocrinology, nephrology, neurology, gynecology, dermatology and ophthalmology are introduced.
