In an attempt to study human gastric epithelial cell proliferation before snd after eradication of Helicobacter pylori (H. pylori), gastric mucosal biopsies from the antrum and body were taken from 30 patients with H. pylori positive gastritis before and 4 weeks after eradication therapy. The regimen used for H. pylori eradication was triple therapy consisting of Metronidazole 750 mg/day, Omeprazole 40 mg/day, and Clarithromycin 600 mg/day for one week. H. pylori status was assessed by culture, histology and rapid urease test. Cell proliferation was studied by immunohistochemical staining of proliferating cell nuclear antigen (PCNA). The proliferative zone was defined as the thickness of PCNA-positive cells between the highest and lowest cells. The number of PCNA-positive cells, proliferative zone and proliferating index were significantly decreased in the antrum and body after eradication of H. pylori. The pattern of the proliferative zone in the antrum was similar to that in the gastric body after eradication of H. pylori. These results suggest that H. pylori infection is associated with increased gastric epithelial cell proliferation. The reduction in epithelial cell proliferation in both the gastric antrum and body after eradication of H. pylori may reduce risk of gastric cancer.
