A CASE OF PYODERMA GANGRENOSUM

The patient with pyoderma gangrenosum is a 34-year house wife. On admission, an ulceration of about 18×14 cm in size was found at the flexor side of her upper limb. The comparison study of histologic findings and electronmicroscopic observation were done. Biopsy of the base and marginal area of the ulceration show diffuse intensive eosinophile, neutrophile, and many large clear cells similar to that of the malignant tumor. In electronmicroscopic observation, the disappearance of the central band of specific granules of eosinophiles, dissolution of the central portion were noted, marked irregular fibroblast proliferation similar to that of the malignant tumor was seen, and irregular intercellular distribution of collagen fibers with unclear cross-striated structure were seen. The dramatic effect of the steroid therapy is thought to be due to suppression of the allergic reaction, eosinophilic activation of bone marrow, and suppression of collagen formation in fibroblasts. A further study at the local allergic reaction is also recommended in this disease.
