SHIFT IN PREVALENCE OF ANTIBODIES TO HEPATITIS A AND HEPATITIS B IN AICHI DISTRICT

Sera obtained in 1968 and in 1979 from healthy individuals randomly selected in Aichi district, were studied for the shift in proportion of two types of antibodies; antibody to hepatitis A virus (anti-HAV) and antibody to hepatitis B virus surface antigen (anti-HBs). Four hundred seventy five sera collected in 1968 and 489 sera in 1979 were rondomized to ensure appropriate age and sex distributions, and were tested for anti-HAV by radioimmunoassay and for anti-HBs by passive haemagglutination. The prevalence rates of anti-HAV and anti-HBs were correlated to age (p<0.001). A logistic graph for 1968 derived from discriminant analysis on age dependent prevalence rates of antibodies to HAV and to HBV showed similar patterns to the graph for 1979, but the latter graph was in 10 years older position as compared to the former, indicating the decrease in the prevalence rates of antibodies to both HAV and HBV. This implies that during this period, there was no epidemic which might have influenced the prevalence rate of antibody. There was no significant sex difference in HAV and HBV infection rates. Socioeconomic factors, such as the availability of safe tap water and sewage (for hepatitis A) and the increase in protein intake (for hepatitis B) seem to have contributed to this decline. Japan has an intermediate prevalence rate of HAV and HBV between Asian and African countries and West European countries such as Sweden and Denamrk. The data of the past eleven years suggest that the prevalence pattern in Japan is gradually approaching that of West European countries.
