Recent increase of coronary sclerosis aroused great interests in surgical treatments for coronary lesions. The results of surgical treatment for coronary sclerosis are revealed by the study of the circulation of local heart muscles. A number of methods so far employed were not satisfactory for the evaluation of surgicl procedure from the viewpoint of the above-mentioned local circulation. Hence, the following experiments Were attempted. Heart muscle clearance method of Na I^<131> was preferred because of easiness for clinical application. Heart muscle clearance of dogs was examined before and after the occulusion of anterior descending branch of left coronary artery. The occulusion resulted in retardation of clearance time, while the release brought it back to normal level. This justifies the use of the method. ECG and histological findings as well as heart muscle clearance were employed for the judgement of surgical procedures of coronary sclerosis. They consist of 21 cases of cardio-pericardiopexy (a), 9 cases of ligation of bilateral internal mammary arteries (b), 14 cases of (a) (b) combined operation (c) and 13 cases of implantation of mammary artery (d). 3 months after the operation, clearance was measured under occulusion of anterior descending branch of left coronary artery. The effects of operation turned out ; 43% of (c) moderate and 21% marked, while 23% of (d) moderate and 46% marked. (d) proved superior to (c). (c) is applicable to severer cases, while (d) is as well clinically applicable with little chance of obstruction of implanted artery, if carried on exact course. Thus heart muscle clearance method was considered to be simple enough for clinical application to the observation of local heart muscle circulation.
