The right subclavian artery as the last branch of the aortic arch is believed to a rare anomaly in the Japanese. One case of this anomaly was found in a Japanese female of 84 years old. The principal findings are summarized as follows. 1. The aortic arch has three branches: truncus bicaroticus, a. subclavia sinistra and dextra. 2. Truncus bicaroticus, arising from the upper surface of the aortic arch, runs upward for a short distance (0.3 cm) along the left tracheal margin to be divided into the left common carotid artery and the right one which runs to the right in front of the trachea. 3. The beginning of the right subclavian artery from the posteromedial surface of the aortic arch is 3.1 cm apart from that of the left subclavian artery. The manifest findings on this artery are: a. a funnel-shaped dilatation in its beginning. b. two sharp bends. The artery runs from its beginning transversely behind the esophagus to reach the first bend at the right posterior. border of the esophagus and then passes up along this border to the lower level of the thyroid gland where the second bend is made. c. marked esophagus compression. Dysphagia lusoria and circulatory disturbances of the right arm expected by the marked esophageal compression and the sharp bends of the artery could not be found in her life. 4. The left subclavian artery, the right and left vertebral arteries, the left vagus nerve and the left recurrent nerve are ordinary. 5. There is no branch of the right vagus nerve that winds around the right subclavian artery. The right nerve innervating the larynx arises directly from the truncus of the right vagus nerve at the lower level of the larynx. This nerve appears to correspond to the right inferior laryngeal nerve.
