Various phases of body water were studied in patients with renal diseases, in comparison with renal function as determined by renal clearance tests. The following observations were made: 1. Various phases of body water in normal, healthy persons were as follows: total body water (T.B.W.), 18.76±1.361/m^2.; extracellular water (E.C.W.), 8.92±0.821/m^2.; intracellular water (I.C.W.), 9.85±0.651/m^2.; circulating plasma volume (C.P.V.), 1.825±0.26 1/m^2.; circulating blood volume (C.B.V.), 3.358±0.6961/m^2.; and circulating blood cell volume (C.B.C.V.), 1.539±0.246 1/m^2. 2. In renal diseases, C.P.V. tended to be either normal or increased, C.B.V. either normal or decreased, and C.B.C.V. decreased. 3. Even though there were fluctuations in various phases of body water, C.P.V. in renal diseases remained within a normal range unless the condition was complicated by cardiac insufficiency. 4. In renal diseases, T.B.W. and I.C.W. were either normal or increased. Patients who were edematous were marked by the increase of T.B.W. and E.C.W. 5. The increase of-T.B.W. paralleled that of E.C.W., and the increase of I.C.W. also tended to parallel that of T.B.W. 6. When plasma colloid osmotic pressure was lowered below 200 mm. H_2O, plasma total protein below 6.5gm/dl., and plasma albumin below 2 gm/dl., there was a marked increase in E.C.W., clearly indicating edema. 7. There was a correlation of significance between C.B.C.V. and glomerular filtration. 8. Plasma Na was normal in many of the patients under study, irrespective of fluctuations in various phases of body water. 9. Study on the relation of renal function to various phases of body water showed that fluctuation in the latter were in conformity with the degree of disturbance to the former, i.e. the degree of disturbance to blood flow in the kidneys and of glomerular-tubular imbalance, resulting in extracellular and intracellular water imbalance. It seemed, however that homeostasis was in operation until disturbance to renal function had become far advanced.
