Community-based Comprehensive Support Centers are institutions for conducting comprehensive support work recommended by the Long-Term Care Insurance Law revision of 2006. Community-based Comprehensive Support Centers are becoming the core of local care as a synthetic care management organization. This paper aims to clarify the activities of certified social workers through a questionnaire about the actual condition of Community-based Comprehensive Support Centers in Hokkaido, Japan. Results suggested that the activities of these centers, such as community network development, comprehensive social work, and advocacy of human rights, which are activities expected from certified social workers are not fully practiced. In terms of the priority of roles, it is given to care and prevention management roles. In comprehensive consultation, the actual conditions grasp by visit activities is performed, and an abbreviation half is considered that suitable support at the house of a client is offered. It is important for Certified Social Workers to perform specialized activities to construct a support network with consideration to the community characteristics and ability for one-stop consultation.
