Drug Susceptibility of Acanthamoeba Isolated from 13 Japanese Patients with Acanthamoeba Keratitis

Purpose: This study was conducted to determine the drug susceptibility of Acanthamoeba organisms isolated from corneal scrapings of patients in Japan and to compare the results with the clinical outcome of each patient. Methods: Acanthamoeba isolates from 13 patients with Acanthamoeba keratitis were enrolled in the drug susceptibility test. Cysticidal effects in vitro were evaluated for the following various drugs: as antimycotic drugs, miconazole and fluconazole; and as antiamoebic drugs, polyhexamethylene biguanide (PHMB) and chlorhexidine gluconate. The relationship between the results of these tests and the clinical course of each patient was also determined. Results: The drug susceptibility tests showed similar results for the 13 isolates; cysticidal effects were obtained with the antiamoebic drugs (PHMB and chlorhexidine gluconate), although antimycotic drugs (miconazole and fluconazole) were not effective at all. The clinical outcome was not in accordance with those results. One of the 4 patients using antiamoebic drugs was not cured and 5 of the 9 patients using only antimycotic drugs (miconazole, fluconazole, itraconazole and/or amphotericin B) were cured. Conclusions: The drug susceptibility of Acanthamoeba isolates from Japanese patients was considered to be uniform. However, the outcome of each patient was not in accordance with those results.
