Japanese society is rapidly aging and will continue to do so for the foreseeable future. This paper examines the provision of health care, medical and welfare services in Japan. A number of conclusions are drawn from the case studies of Akashi city in Hyogo Prefecture and Yamato-cho in Niigata Prefecture, supplemented by interviews with workers in the field and citing literature and media reports where relevant. Since the elderly are more likely to require both health care and welfare services at the same time, the virtues of combining health care, medical and welfare services have been extolled for some time. Yamato-cho represents a leading example of the approaches that are being tried out throughout Japan. Health care, medical and welfare services at both the municipal and national levels are physically combined in the same location, and the three services are closely integrated in terms of operation and procedure. This arrangement has proven highly successful, although this is to some extent attributable to the town's reasonably small population of 15,000. At Akashi city, a medium-sized city about 20 times the size of Yamato-cho, the integration of the three services has taken a different form. Health care, medical and welfare services are coordinated in a hierarchical system operating at three levels: the region as a whole, junior high school districts and elementary school districts.
