<原著>合衆国保健医療の企業化と医療管理 : マサチューセッツ州におけるマネジドケアと病院連合体を事例に

The author had an opportunity to investigate healthcare-related organizations in Massachusetts, USA, from April 29 to May 5, 2001. This report evaluates the current situation of the U.S. healthcare system from the aspect of the Corporatization Theory, characterizing the dynamic alteration in the U.S. healthcare as a shift to a corporate mode of financing and delivery and the diminution of professional power. The findings are as follows: (1) Faced with industry-wide depression, several big healthcare organizations began to merge, acquire, or cooperate with others, so that the concentration of healthcare industries has proceeded rapidly. (2) Widespread backlash against the insurer-controlling managed care organizations invoked Massachusetts State's managed care legislation resulting in the protection of physicians' discretion to their patients. (3) Instead of the insurers, the newly formed hospital alliances have engaged in standardizing medical practices, and by using the information technology, they have achieved higher cost efficiency. These findings suggest the need for further research concernig the role of hospital-controlling managed care organizations and the possible standardization in medical practices. However, these cost-efficiency-centered changes can erode fairness in the healthcare system, and this is the critical point that the Corporatization Theory is concerned with.
