Comparison of Guided Tissue Regeneration Procedure to Conventional Flap Surgery in Treating Intrabony and Class II Furcation Defects, and Analysis of Factors Affecting the Prognosis Following GTR

本研究の目的は(1)垂直性骨欠損と下顎根分岐部II級病変を有する部位に対して施術した歯周組織再生誘導法(GTR法)の臨床効果を対照群として設定したフラップ手術の効果と無作為割付した系で比較検討すること,(2)GTR法の臨床効果に影響を及ぼす因子について多変量解析法を用いて包括的に評価することにある。被験者は,35歳から60歳までの成人性歯周炎と診断され,各人につき1部位,垂直性骨欠損または下顎根分岐部II級病変を有する80名を対象とした。従って被験部位は,全部で垂直性骨欠損を有する60歯,下顎根分岐部II級病変を有する20歯となった。歯周基本治療を完了後,これらの被験部位は年齢,性別,歯種,欠損形態を考慮した上で,実験群(GTR群)と対照群(フラップ手術群)に無作為に割付けられた。術後,創傷部の安静と徹底的な感染のコントロールを行い,6ヵ月目および1年目に臨床評価を行った。その結果,垂直性骨欠損を有する部位では,GTR群,フラップ手術群ともにベースラインと比較して6ヵ月目および1年目で,歯周ポケット深さ,垂直的付着の獲得量,辺縁歯肉の退縮量,X線上の骨再生量に有意に改善が見られた。さらに,GTR群はフラップ手術群よりも術後6ヵ月目,1年目の垂直的付着の獲得量,辺緑歯肉の退縮量,骨再生量で統計学的に有意な差をもって改善した。根分岐部II級病変を有する部位では,GTR群においてのみ,ベースラインと比較して6ヵ月目および1年目の評価で,歯周ポケット探さ,垂直的付着,ならびに水平的付着の獲得量に有意に改善が見られた。またGTR群はフラップ手術群と比較して術後6ヵ月目,1年目の水平的付着の獲得量に有意な改善が見られた。膜の露出は垂直的付着獲得量,骨再生量,水平的付着の獲得量と負の関係を示した。GTR法の結果に及ぼす因子について多変量解析を行った結果,ベースライン時の歯周ポケット深さ,骨吸収量,水平的付着の喪失量が,術後6ヵ月目の垂直的付着獲得量,骨再生量,水平的付着の獲得量の予測値となることが示された。さらに術後6ヵ月目の垂直的付着の獲得量,骨再生量,水平的付着の獲得量が,術後1年目の垂直的付着の獲得量,骨再生量,水平的付着の獲得量の予測値であることが示された。故に,垂直性骨欠損を有する部位ではベースライン時の歯周ポケット深さと骨吸収量が,また根分岐部II級病変を有する部位では,ベースライン時の水平的付着の喪失量が歯周組織再生量を予測する因子であり,術後6ヵ月間で最終的な組織再生量が予測し得るものと考えられる。
