Paralytic Reactions Occurring in Essential Hypertension Patients after Sublingual Nitroglycerin Administration and during Treatment with Nifedipine and β-blockers

The depressor reactions in response to nitroglycerin (TNG; sublingual; 64 patients), nifedipine (NIF; chronic therapy, 6〜8 weeks; 63 patients), and β-blockers (6〜12 weeks; 66 patients) were observed in essential hypertension patients. Hemodynamic analyses were performed by the Wezler-Boeger method, and the depressor reaction was classified on the basis of the direction of changes in cardiac output (CO) and total peripheral resistance (TPR) into a "paralytic (P) reaction", "tone-decreasing (Entspannungsreaktion [E]) reaction", and "tone-increasing (Anspannungsreaktion [A]) reaction". The P reaction, in which both TPR and CO decrease in association with the blood pressure reduction, was observed in approximately 13% of those treated with TNG and NIF, and in approximately 13% of those treated with β-blockers. There were no major differences in the E and A reactions. According to age group, the P reaction was observed more frequently in the 60 years and over group treated with TNG (17.4% vs 9.8%) and NIF (23.8% vs 7.1%) than in the under 60 group. There were no differences with the β-blockers. The development of the paralytic reaction must be borne in mind whenever sublingual TNG or NIF therapy are used to treat elderly patients.
