A Case of Adult Onset Still's Disease with High Level of Serum Ferritin and Liver Siderosis

We report a case of adult onset Still's disease (AOSD) whose serum ferritin level increased markedly at onset and decreased with the treatment of prednisolone. Liver biopsy performed at onset demonstrated siderosis due to excessive administration of medical iron in the past, and bone marrow aspiration revealed increased iron in macrophages and decreased sideroblasts. With decreased serum ferritin level liver biopsy and bone marrow aspiration were repeated to investigate the relationship between the body storage iron and the serum ferritin level. Liver biopsy presented unchanged moderate siderosis, while bone marrow disclosed normal. Marked elevation of serum ferritin in the present case didn't directly reflect the reticuloendothelial (RE) storage iron whether the cause of storage is iron overload or inflammation.
