• Ischemic heart disease known as having many puzzling discrepancies among the clinical manifestations and pathological findings, was studied based on the anatomical characteristics of the intramyocardial coronary branches and their behavior at the development of the ischemic lesion. Utilizing postmortem angiographic technic, transparent preparation and histological examination, one hundred thirty-eight hearts including fortyfive cases of myocardial infarction were examined. The age of ischemic lesion was judged by the criterion proposed by Mallory. At the initial stage, infarcted area appeared to be avascular. Three and four weeks later, the area became hypervascular secondary to neovascular formation of arterioles and dilatated capillaries. At the end stage, 8-10w. after onset, the lesion was replaced by dense fibrous tissue forming scar, presenting hypovascular figure accompanied with enlarged tortuous vessels in the peripheral zone. So-called no-reflow phenomenon was observed at the very early stage of the ischemic lesion. In addition to the intercoronary anastomoses well documented by a number of investigators, remarkable compensatory ability of the intramyocardial branches may modify and minimize the clinical symptoms and progression of the disease. Comparative analysis of the clinical and pathological findings obtained in this study, suggests that repeated episodes of coronary insufficiency have been taken place until the occurrence of the fatal myocardial infarction.
