P Wave in Primary Pulmonary Hypertension

To clarify the pathophysiology of primary pulmonary hypertension (PPH), the P waves of 83 patients from hospitals throughout Japan were investigated concerning the relation to pulmonary hemodynamics, arterial blood gas data and the prognosis. On the whole, the AP was 64.0+15.2°, the amplitude of the P wave in lead II (PII) was 1.7+0.7mm, the duration of PII 0.08+0.01 sec and the P terminal force in V_1 (PTFV_1) -0.03+0.04mm sec. The absolute value of PTFV_1 in the deceased group was significantly higher (P<0.02) than that in the surviving group, and there was a significant negative correlation between the survival time and AP (p<0.05). There was no relation between P waves and the cardiopulmonary pressures, but there was a significant negative correlation between the amplitude of PII and the cardiac index (p<0.05) and a positive correlation between PTFV_1 and PaO_2 (p<0.05). In cases with AP of more than 60, the cardiac index was lower (p<0.005) than in cases with lower AP values. In cases with a PII amplitude of 2mm or more, the right atrial mean pressure was higher (p<0.001) than in cases with lower PII values. In cases with PTFV_1 of -0.03mm sec. or less, the cardiac index had lower values (p<0.02) than in cases with PTFV_1 of more than -0.03mm sec. From the above results, it is concluded that in patients with PPH, overall evaluation of AP, PII and PTFV_1 is useful for pulmonary hemodynamics and prognosis evaluation.
