Clinical Significance of Random Mucosal Biopsy in Primary Superficial Bladder Cancer

Objectives. We examined the risk factor of positive rate of random mucosal biopsy in primary superficial bladder cancer, and how random biopsy is associated with prognosis. Methods. Between April 1990 and December 2000, in 77 patients with newly diagnosed superficial bladder cancer random mucosal biopsy were performed at 7 normal-looking urothelium at the time of TUR-Bt. The risk factor of positve rate of random biopsy and the relationship between the results of random biopsies and the non-recurrence rate was examined. Results. Of the 77 cases 29 demonstrated abnormal findings in biopsy (20 dysplasia, 9 CIS). Our results showed that the positive biopsy rate in biopsy was significantly higher in patients with a high-grade cancer. The non-recurrence rate were not significantly different between the positive biopsy group and the negative biopsy group in all cases. Furthermore we divided all patients into two groups, which are the intravesical instillation therapy group and the non-intravesical instillation therapy group, and then we examined the non-recurrence rate between the positive biopsy group and the negative biopsy group in each of these two groups. No significant difference was observed in the non-recurrence rate between the positive random biopsy group and the negative biopsy group. Conclusion. Taking routine random mucosal biopsy of normal-looking urothelium at the time of TUR from patients with primary superficial bladder cancer has not much practical value because the non-recurrence rate were not significantly different regardless of the results of random biopsy. It is considered that the number of cancers, size, grade and stage of the primary cancer, which are regarded as prognostic factors, are sufficient indicators for selection of postoperative adjuvant therapy for primary superficial bladder cancer.
