Early Diagnosis of Post-Transfusion Graft-versus-Host Disease : A Case Report

A viable treatment for PT-GVHD, a fatal complication of blood transfusion, has not yet been identified. Since recovery from GVHD which has advanced beyond the acute stage is unlikely, early diagnosis of PT-GVHD is essential for successful treatment of this disease. The case of PT-TGVHD presented here yielded important information about early diagnosis of the development of PT-GVHD. The patient was diagnosed-as having PT-GVHD by means of genetic analysis of a chimera of microsatellite DNA polymorphisms, skin biopsy and the CD 4/CD 8 ratio. However, these methods are time-consuming. In this paper, we propose that PT-GVHD should be suspected if clinical features suggestive of acute GVHD, such as fever of unknown origin and skin rash, are observed 1 to 2 weeks after blood transfusion.
