体表微小振動(minor tremor)とその変化という視点よりみた口角結節の研究

Minor Tremor (abbreviated by MT), first described by Rohracher, H. in 1946 has been reported frequently on the the human thenar and animal abdominal-wall portion, infrequently on the human facial portion, but not on such a special facial portion as modiolus. In this study, the last problem was studied. Kamogawa has indicated that MT at any portoin on the face is altered thus serving as an index of reflex behavior in response to oral afferent stimuli. Standing on the same experimental base, MT on the special modiolus site where all the expression muscles send more or less their muscle fiber bundle to this place was investigated. Modiolus was manually determined by palpation as a thickened point about 5mm lateral to the oral angle and then about 5mm superior. Original records, integrated records and histogram manifestation of modiolus MT were collected and examined in 165 persons (healthy persons and dental patients). In or under such slight dental pain grade as general facial MT presents only reduction in the most dominant band, modiolus MT shows simultaneously the reduction in the most dominant band and the increase in the next dominant one (s), consequently resulting in double heads (θα or αβ_1 pattern) or three heads ( θαβ_1 pattern). These changes can be readily demonstrated only in modiolus MT as a response. Resting MT histograms at the modiolus are site-specfic in the aspect that the histogram has a marked unipolarity such as α dominancy or θ dominancy. MT tends to show α dominancy with increasing soft tissue ratio under the electrode portion, as previously written. In the state with clear continuous dental pain, both general facial MT and specific modiolus MT show a responsive MT pattern which rises to β_1 band. The most striking features of the modiolus MT response can be demonstrated in the borderline case. In this case, the mouth is usually healthy and has nothing pathologic while it is by chance tooth-or temporomandibularjoint-ached, hence being not wholly normal but near normal. The resting pattern of the modiolus MT in the borderline cases was characterized by repetiton of the normal histogram spectrum of monopolar, one dominant pattern and responsive histogram spectrum of reduction of all bands, in the rate of 1 : 2,1 : 3 or 1 : others dependent upon the pathophysiological state. Now the factor to determin the rate remain obscure These states were equivalent to the undulation MT figure in the very early or final disease process. In conclusion, the human modiolus is not only on anatomically specific site at which almost all the expression muscles send their partial muscle bundle but also pathophysiologically specific site at which the MT change can expose the very early or the most final response to orodental pain.
