Extracellular Matrix in Diabetic Vascular Wall : Implication in Accelerated Atherogenicity

Diabetes mellitus is a known accelerating factor for atherosclerotic vascular diseases. The cause of highly proliferative property in diabetic vascular smooth muscle cells(SMC)includes overexpression of the platelet-derived growth factor(PDGF)β-receptor. Fibronectin, an extracellular matrix protein, significantly activated the PDGF β-receptor promoter in cultured rat SMC. The fibronectin-induced transcriptional activation was dependent on activity of focal adhesion kinase and MEK1 / ERK pathway. Introduction of a dominant negative mutant receptor for TGF-β into balloon-injured rat carotid artery inhibited expression of PDGF β-receptor as well as fibronectin in the arterial wall. Moreover, growth and migratory signals by PDGF were synergistically enhanced by an extracellular matrix protein osteopontin(OPN). Exposure of SMC to high glucose enhanced OPN secretion via hexosamine / protein kinase C pathways. The alteration of SMC phenotype characterized by modified expression of extracellular matrix components seems to be an important determinant of atherogenicity in diabetes.
