自然発症糖尿病ラットにおける上顎側方拡大後の正中口蓋縫合の修復過程に対するBasic Fibroblast Growth Factor(b-FGF)の効果

It is well known that the wound healing is delayed in diabetic patients. Recently, it has been reported that basic Fibroblast Growth Factor (b-FGF) promotes the bone fracture healing as a result of stimulating and proliferating the fibroblasts and osteoblasts in local administration. The purpose of this study is to examine the process of collagen fiber and new bone formation in the midpalatal suture after maxillary expansion in the diabetic rats injected with b-FGF histopathologically. One hundred spontaneously diabetic (BB/W) male rats similar to human Insulindepended juvenile diabetes mellitus (IDDM) as experiments and fifty male Wistar strain rats as control, aging from 12 to 14 week-old, were used in this study. Throughout the experiment these BB/W rats were controlled every day with intravenous human insulin injection of 2-6U after diabetic attacks. Body weight, blood glucose and blood were evaluated after experimental periods. All animals were divided into 12 groups comprising of 4 expansion groups in the Wistar strain rats (normal control group), 4 expansion groups injected with b-FGF in the BB/W (FGF groups) and 4 expansion groups without b-FGF (control groups). An expansion device was installed between maxillary incisors for two days in all groups. The expansion space was retained throughout the experimental period after the treatment. The b-FGF was injected submucously in the region of midpalatal suture area at only first day of retention. The animals were sacrificed at 1,3,5 and 7 days after expansion. The specimens were examined with Hematoxylin Eosin stain, Azan Mallory stain and immunohistochemical stain using the PCNA and FGF-receptor antibody. The Ca and P concentration in new bone were measured by EPMA. The results were as follows : 1. In the FGF groups, many fibroblasts, osteoblasts and blood vessels were actively observed in the sutural space after maxillary expansion as compared with the control groups. 2. The positive cells of PCNA were observed in all groups. The normal control and the FGF groups were significantly observed as compared with the control groups at the first and third days of retention. 3. FGF-receptors were observed in all groups and gradually decreased day by day. FGF-receptor in the normal control groups and the FGF groups were observed numerously as compared with the control groups. 4. In EPMA analysis, the normal control groups and FGF groups showed significantly higher level of Ca and P concentration in new bone area than the control groups at the seventh day of retention. There were no significant difference between the normal control groups and the FGF groups in the different days. 5. As for the new bone formation, the normal control groups and the FGF groups showed a significant increase of new bone as compared with the control groups after the third day of retention. However there were no significant difference between the normal control groups and the FGF groups. From these observation, it was revealed that the bFGF stimulated and enhanced the the cell proliferation and activity of fibroblasts and osteoblasts in the spontaneously diabetic rats. Consequently, It was suggested that the b-FGF was found to be very effective in osteogenesis and for the stabilization of the midpalatal suture areas after maxillary expansion, if the control of blood glucose levels is good in IDDM patients.
