Myasthenia gravis (MG) is an immune disorder of the neuromuscualr junction in which antibodies against the nicotinic acetylcholine receptor (AChR) cause a reduction of AChR, resulting in defective neuromuscular transmission. Plasmapheresis (PP) with immunosuppressive drug therapy has been used to treat severely affected patients. PP appears to be effective and shorten the duration of the MS crisis. Clinical improvement induced by PP is thought to be a result of the accumulation of newly synthesized AChR due to a decrease in the rate of antibody-mediated AChR loss. Amelioration of the functional block of AChR may explain, at least partly, the rapid recovery of muscle strength after PP, which occurs faster than the turnover of AChR at the end-plate. While several uncontrolled trials showed that PP can provide rapid but relatively short-term improvement, very few controlled trials have been carried out. Randomized controlled trials to determine a more efficient regimen would be beneficial for the treatment of MG.
